胃癌行全胃切除术患者术后生存质量的质性研究

Qualitative research of life quality in gastric cancer patients after total gastrectomy

目的深入了解胃癌全胃切除术患者对生存质量的真实评价,从各个层面探讨患者术后生存质量的影响因素,为提高其生存质量提供理论依据。方法采用质性研究中现象学研究法,对10例全胃切除术后患者进行深度访谈,对获得的资料进行分析,从生理、情感、家庭/社会3个角度来提炼主题。结果共提炼出8个主题,分别为:并发症困扰、化疗反应强烈、焦虑、自责、需求健康信息、否定自我价值、需要家庭支持及社会适应能力下降。结论全胃切除术后患者的生存质量受并发症、化疗反应和负性情绪的影响,需要家庭和社会的支持,护理人员应掌握患者存在的问题,采取针对性的护理措施,以提高患者术后的生存质量。

Objective To investigate the factors influencing the living status of gastric cancer patients after total gastrectomy.Methods We interviewed 10 patients after total gastrectomy in depth by the qualitative phenomenological method and analyzed the qualitative data to summarize themes physiologically,emotionally and domestically/socially.Results Totally,8 themes were worked out including worry about complications,fierce reaction from chemotherapy,anxiety,self-reproach,needs of health knowledge,negation of self value,needs of family support and decreased social adaptability.Conclusion The living quality of patients with total gastrectomy is influenced by complications,chemotherapy reactions and other negative emotions.They need family and social supports.Therefore,nurses should take pertinent nursing strategies so as to improve the life quality of patients.
