摘要
目的探讨血液透析(HD)、血液透析滤过(HDF)、血液透析联合血液灌流(HD+HP,HDP)对慢性肾衰竭(CRF)患者认知功能、生活质量及营养状况的影响。方法78例CRF患者采用分层随机化方法分为HD组、HDF组、HDP组,治疗前、后应用简易智能状态检查(MMSE)、威斯康星卡片分类测验(WCST)、SF-36健康调查量表进行认知功能、生活质量调查及营养指标检测并分析。结果治疗3、6月后三组MMSE[分别(22.62±3.85)分、(25.10±2.26)分、(25.35±2.44)分及(25.87±4.19)分、(25.10±2.26)分、(27.19±2.23)分]、WCST各指标较治疗前差异有统计学意义(P〈0.05);治疗1,3,6月后3组SF-36躯体健康(PF)、躯体角色功能(RP)、躯体疼痛(BP)、总体健康(GH)、社会功能(SF)维度及营养状况指标较治疗前差异有统计学意义(P〈0.05);HDF、HDP组与HD组差异有统计学意义(P〈0.05),而HDF组与HDP组比较差异无统计学意义(P〉0.05)。结论HDF与HDP对CRF患者远期认知功能、生理健康及营养状况有明显的影响,且优于HD;HDF与HDP无明显差别。
Objective To assess the effect of hemodialysis ( HD), hemodiafihration (HDF) and hemodial- ysis (HD) and hemoperfusion (HDP) on the quality of life, cognitive function and nutritional status in chronic renal failure (CRF) patients. Methods All patients were randomly divided into HD, HDF and HDP group. Mini-mental state examination( MMSE ), Wisconsin card sorting test ( WCST), SF-36 were measured for cognitive function, quality of life and nutrition indicators test. Results After 3,6 months treatment, MMSE ( respectively ( 22.62±3.85 ), (25.10 ± 2.26 ), (25.35 ± 2.44) and (23.87± 4.19), (25.10 ± 2.26), (27.19 ±2.23 ) ), WCST indi- cators were statistically significant in the three groups (P 〈 0.05 ) ; after 1,3,6 months treatment, the PF, RP, BP, GH, SF and nutrition indicators were significant difference too(P〈 0.05) ; HDP and HDF, HD groups were statisti- cally significant (P 〈 0.05 ). Conclusion HDF and HDP obviously influence on long-term cognitive function, phys- ical health domains and nutritional status in CRF patients, and superior to HD.
