骨科植入物及器械库存式管理模式降低手术室护理人力成本

Application of Standardized Administrative Procedures for Orthopedic Implants to Reduce Nursing Labor Costs

目的探讨一种全新的骨科植入物及器械标准化流程管理模式降低手术室护理成本及间接提高手术质量的优越性。方法以2008年1-12月开展的全髋关节置换术及脊柱内固定术为对照组,采用临时外送植入物及器械管理模式;以2009年1-12月的相应手术为试验组,采用库存式植入物及器械管理模式。通过计算护理人力耗用工时来比较两种管理模式下护理人力成本投入的差异。结果不论是全髋关节置换术还是脊柱内固定术,试验组的术前、后程序耗时及总人力耗时均明显少于对照组,差异有统计学意义(P<0.01)。结论采用标准化库存式骨科植入物及器械流程管理模式减少了护理人员的工作量、提高了工作效率,从而有助于降低手术室护理人力成本、提高医生满意率、降低感染率,并由此提高手术质量。

Objective To investigate the advantages of the standardized administrative procedures for or- thopedic implants to reduce the nursing labor costs as well as indirectly improve the quality of medical services. Methods The patients undergoing total hip arthroplasty and spinal fixation from January to De- cember 2008 served as control group to carry out temporary delivery implants and device management model. The patients undergoing the appropriate surgiries from January to December 2009 served as control group to carry out inventory-type implants and device management model. By calculating the consumption of man-hours of nursing manpower, the nursing labor costs were compared under the two management models. Results The procedure time-consumption and the total human time-consumption before and after surgery in the experimental group were significantly less than those in the control group undergoing total hip arthroplasty or spinal fixation （P〈 0. 01 ）. Conclusion The standardized administrative procedures significantly reduces the nursing labor costs,and improves the work efficiency, thus helping to reduce operating room nursing labor costs, improving physicians＇ satisfaction, lowering infection rates and thus improving the quality of surgery.
