摘要
目的:探讨多层螺旋CT平扫及多平面重建对十二指肠球后溃疡急性穿孔的诊断价值。方法:回顾性分析经手术证实的11例十二指肠球后溃疡急性穿孔患者的多层螺旋CT表现。结果:11例球后溃疡穿孔均位于十二指肠降部近侧,近球顶部(上曲部)4例,近十二指肠乳头7例。后壁穿孔7例,内侧壁穿孔4例。穿孔大小为0.5～1.5 cm。多层螺旋CT表现:局部肠壁缺损,造影剂外漏;穿孔处周围腹膜后积液积气,蜂窝组织炎;局部消化管壁增厚模糊。结论:多层螺旋CT平扫及多平面重建方便快捷,可清晰显示球后溃疡穿孔的直接征象及继发改变,准确做出定位定性诊断,可作为首选检查方法。
Objective:To explore the Diagnostic value of multi-slice spiral CT(MSCT) conventional scan and its multi-planar reformation(MPR)in postbulbar duodenal ulcer acute perforation.Methods:Retrospectively analysis the outcome of the multi-slice spiral CT of 11 patients whose postbulbar duodenal ulcer acute perforation has been confirmed by operations.Results:All the 11 cases of postbulbar duodenal ulcer acute perforation are proximal to the descending part of the duodenum,in which 4 cases are near the spherical top and the rest are proximal to duodenal papilla,meanwhile there're 7 of the posterior wall perforation in the 11 cases,and 4 of the medial wall perforation.The perforation size is about 0.5~1.5 cm.The MSCT manifestations: local intestinal wall defects and the contrast agent leakage;surrounding perforation retroperitoneal space accumulates liquid and gas,cellulitis;local gastrointestinal wall is thicken and fuzzy.Conclusion:MSCT conventional scan and its MPR are convenient and efficient,can clearly display the postbulbar ulcer perforation direct sign and secondary changes,to make an accurate localization and qualitative diagnosis,can be used as the preferred inspection method.
