摘要
胰十二指肠切除术是治疗胰头、十二指肠壶腹周围肿瘤的经典术式,而该术式涉及的脏器多、手术难度高、时间长、创伤大,故术后并发症多。自1935年Whipple首先开展这一术式以来,随着外科技术操作的不断提高和改进、围手术期治疗的改进、术式的进一步完善,以及影像学、分子生物学、介入医学、检验医学等的不断发展,使PD术后并发症及死亡率已明显下降。尽管PD术的病死率已经大幅度下降,但术后并发症仍然高达30%～65%。该文就PD术后并发症及其处理进行综述。
Pancreatoduodenectomy（PD） is the classic surgical for treatment of pancreatic head、peri-ampullary tumors,with many organs involved,surgery difficult and a long time,serious trauma in surgical procedures,which lead to so many complications in PD.Since Whipple carried out this operation first in 1935,along with the operation and surgical techniques improvement,improved perioperative treatment,further improvement of surgical and imaging,molecular biology,interventional medicine,laboratory medicine,etc,the complications and mortality of patients with PD have decreased significantly.Although the mortality of patients with PD have decreased significantly,complications are still as high as 30% to 65%.Complications and treatment of PD are reviewed in this paper.
