血清抵抗素与动脉粥样硬化性脑梗死患者危险因素与病变程度的关系研究 被引量:1

摘要 目的探讨动脉粥样硬化性脑梗死患者血清抵抗素水平与脑梗死危险因素以及临床神经功能缺损程度、脑梗死体积等的相关性。方法收集临床诊断为动脉粥样硬化性脑梗死患者100例及健康对照者90例,分析动脉粥样硬化性脑梗死患者的病变程度及危险因素与血清抵抗素的关系,同时对轻、中、重患者及梗死灶体积小、中、大患者依次分析其与血清抵抗素的关系。结果脑梗死组腰臀比(WHR)、收缩压(SBP)、舒张压(DBP)、血清抵抗素、瘦素(LP)、胰岛素(FINS)、空腹血糖(FBG)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、超敏C反应蛋白(hs-CRP)、同型半胱氨酸(HCY)均高于对照组,高密度脂蛋白胆固醇(HDL-C)低于对照组,差异有统计学意义(P<O.05)。抵抗素与BMI、SBP、LP、FINS、FBG、LDL-C、hs-CRP、HCY呈正相关(P<0.05),而与HDL-C呈负相关(P<0.05)。轻、中、重型三组间,血清抵抗素浓度差异有统计学意义;小、中、大梗死三组间,血清抵抗素浓度差异有统计学意义。结论血清抵抗素是动脉粥样硬化性脑梗死重要的影响因素,不但参与糖脂质代谢紊乱的发生,而且与高血压、肥胖以及动脉粥样硬化炎性反应等有关。急性脑梗死患者血清抵抗素水平与神经功能受损程度、脑梗死灶体积大小等相关。 Objective To investigate the atherosclerotic cerebral infarction in patients with serum resistin levels and risk factors for cerebral infarction and the clinical neurological deficits,infarct volume and other relevant.Methods of clinical diagnosis of atherosclerosis of the 100 patients with cerebral infarction and healthy controls,90 patients were analyzed atherosclerotic lesions in patients with cerebral infarction and the risk factors and the relationship between serum resistin,while light,medium,weight and infarct size in patients with small,medium and large were followed by analysis of its relationship with serum resistin.Results of cerebral infarction waist-hip ratio(WHR) ,systolic blood pressure(SBP) ,diastolic blood pressure(DBP) ,serum resistin,leptin(LP) ,insulin(FINS) ,fasting blood glucose(FBG) ,total cholesterol(TC) ,triglyceride(TG) ,low-density lipoprotein cholesterol(LDL-C) ,high-sensitivity C-reactive protein(hs-CRP) ,homocysteine(HCY) were higher,high-density lipoprotein cholesterol(HDL-C) lower than the control group,the difference was statistically significant(P O.05) .Resistin and BMI,SBP,LP,FINS,FBG,LDL-C,hs-CRP,HCY was positively correlated(P 0.05) ,but not with HDL-C was negatively correlated(P 0.05) .Light,medium and heavy among the three groups,serum resistin concentrations significantly;small,medium and large infarction among the three groups,serum resistin levels significantly.Conclusions Serum resistin is atherosclerotic cerebral infarction an important factor,not only in the occurrence of lipid metabolism of sugar,but also with hypertension,obesity and atherosclerosis,the inflammatory response and so on.Patients with acute cerebral infarction and serum resistin level of neurological impairment,infarct volume size related.
