经阴道彩色多普勒超声对异位妊娠及宫内早孕妊娠黄体的对比研究

Transvaginal Color Doppler Sonographic Comparison of Corpus Luteum in Early Intrauterine and Ectopic Pregnancy

目的探讨正常宫内早孕及异位妊娠黄体的经阴道彩色多普勒超声表现。资料与方法对比分析经手术及病理证实的异位妊娠(A组,64例)及宫内早孕(B组,71例)妊娠黄体的经阴道彩色多普勒超声表现。结果黄体类型:A组低回声型33例,厚壁囊肿型19例,薄壁囊肿型6例,薄壁囊肿内部回声点型5例;B组低回声型39例,厚壁囊肿型21例,薄壁囊肿型5例,薄壁囊肿内部回声点型6例;两组回声类型比较,差异无统计学意义(x2=0.663,P=0.882)。A、B组黄体平均直径分别为(1.70±0.51)cm和(1.84±0.37)cm,差异无统计学意义(t=-1.887,P=0.061)。A、B组黄体阻力指数(RI)分别为(0.48±0.09)和(0.45±0.06),差异有统计学意义(t=2.605,P=0.011)。结论经阴道彩色多普勒超声观察黄体回声类型及大小不能反映妊娠黄体功能,而血流显像对估测黄体功能有较高的价值。

Purpose To evaluate the characters of corpus luteum in ectopic pregnancy （EP） and early intrauterine pregnancy with transvaginal color Doppler sonography.Materials and Methods Transvaginal sonographic findings of corpus luteum in 64 cases with ectopic pregnancies confirmed by surgery and pathology （group A） and 71 normal pregnant women （group B） were retrospectively reviewed.Results The corpus luteum could be divided into four types according to sonographic appearance：hypoechoic solid （group A,n=33;group B,n= 37）,cyst with thick wall （group A,n=19;group B,n=21）,cyst with thin wall （group A,n=7;group B,n=5）;cyst with thin wall and internal echogenic dots （group A,n=5;group B,n=6）.There was no significant difference in the configuration between the two groups （x 2 =0.663,P=0.882）.The average size of corpus luteum [group A （1.70±0.51） cm;group B （1.84±0.37） cm] was not different between the two groups （P 0.05）,but the resistance index （RI） in the corpus luteum arteries in group A was （0.48±0.09） compared with （0.45±0.06） in group B （t=2.605,P=0.011）.Conclusion The configuration and size of corpus luteum do not correlate with the functions of the corpus.However the resistance index is valuable in assessing the corpus function.
