基于问题提出的护理干预对全髋关节置换病人术后康复的影响

The Influence of Problem-Based Nursing Intervention on the Recovery in Patients after Total Hip Arthroplasty

目的评价基于问题提出的护理干预对全髋关节置换病人术后恢复情况和生活质量的影响,探讨建立促进病人康复的护理干预模式。方法将100例全髋关节置换病人随机分为Ⅰ组和Ⅱ组,各50例。Ⅰ组采用一般护理干预。Ⅱ组采用基于问题提出的护理干预。观察两组日常生活能力、肌力、关节活动情况、住院天数、并发症的发生率。调查病人围术期关注的常见问题,运用循证医学建立完善的目标护理模式。结果术后21天、3个月生存质量评分及Harthel指数Ⅱ组高于Ⅰ组。Ⅱ组病人满意程度高,出院时肌力的恢复情况、关节活动情况优于Ⅰ组。住院天数Ⅱ组低于Ⅰ组。结论运用循证医学建立基于问题提出的护理干预模式,能减少全髋关节置换术后并发症的发生,促进术后康复,提高生活质量。

Objective To evaluate the influence of problem-based nursing intervention on the recovery and quality of life in patients after total hip arthroplasty,and to investigate establishing nursing intervention mode to promote recovery. Methods 100 patients underwent total hip arthroplasty and were divided into two groups randomly Group Ⅰ（Conventional nursing intervention group） and Group Ⅱ（Problem-based nursing intervention group）.Ability of daily life,muscle strength,articular movement,hospital stay and incidence of complication were observed.Perioperative frequently asked questions were investigated and perfect target nursing mode was established using evidence-based medicine. Results Life quality score and Harthel index of 21 days and 3 months after operation were higher in group Ⅱ.The satisfaction rate,recovery of muscle strength and articular movement in group Ⅱ were better than that in groupⅠ.Hospital stay of patients in group Ⅱ was shorter than that in group Ⅰ. Conclusion Establishing problem-based nursing intervention mode with evidence-based medicine is helpful in reducing complication,promoting recovery and improve quality of life in patients after total hip arthroplasty.
