不同程度颈动脉狭窄的脑梗死患者生化指标的变化 被引量:7

摘要 目的观察不同程度颈动脉狭窄的脑梗死患者生化指标的变化,为脑梗死患者颈动脉狭窄的发生、发展过程提供实验依据。方法回顾性分析100例颈动脉狭窄的脑梗死患者(脑梗死组)的临床资料,所有患者依据超声多普勒血流显像特点分为3组:第1组为颈动脉狭窄正常或狭窄<50%,共34例;第2组为颈动脉狭窄50%~69%,共33例;第3组为颈动脉狭窄>70%至接近闭塞,23例。选择同期在本院体检的健康成年人为对照组,50例。分别测定各组血清同型半胱氨酸(HCY)、总胆固醇(TC)、三酰甘油(TG)和葡萄糖(GLU)水平,并进行比较。结果脑梗死组HCY、TC、TG和GLU水平均高于对照组,两组比较差异有统计学意义(P<0.05)。随着单侧颈动脉狭窄狭窄程度的增加,HCY、TC、TG、GLU水平也增高,三组比较差异有统计学意义(P<0.05);随着双侧侧颈动脉狭窄狭窄程度的增加,TC、TG也增高,但三组比较差异无统计学意义(P>0.05),血清GLU和HCY水平增加,且三组比较差异有统计学意义(P<0.05)。结论血清HCY、TC、TG和GLU水平是颈动脉狭窄发生、发展过程的重要危险因素。 Objective To observe biochemical changes in cerebral infarction patients with different degrees of carotid artery stenosis,and supply experimental basis for the development and progression of carotid artery stenosis.Methods Clinical data of 100 patients of cerebral infarction with carotid artery stenosis were retrospectively analyzed,which were divided into three groups by ultrasound Doppler flow imaging features.Group 1:34 patients,degree of carotid artery stenosis was normal or less than 50%;group 2:33 patients,degree of carotid artery stenosis was 50% to 69%;group 3:23 patients,degree of carotid artery stenosis was 70% to total occlusion.Control group:50 healthy people who received physical examination in the hospital during the period.Each group was measured by serum homocysteine(HCY),total cholesterol(TC),triglyceride(TG)and glucose(GLU),results were compared with each other.Results Cerebral infarction HCY,TC,TG and GLU levels were higher,the difference was statistically significant(P〈0.05).HCY,TC,TG,GLU levels increased along with unilateral carotid stenosis increase,the difference among the three groups was significant(P〈0.05);TC,TG also increased with the bilateral side of the carotid stenosis increase,but the difference among the three groups was not significant(P〉0.05),Serum GLU and HCY levels increased,and the difference between the three groups was statistically significant(P〈0.05).Conclusion HCY,TC,TG and GLU are major risk factors in the occurrence and development of carotid artery stenosis.
