多肿瘤标志物蛋白芯片检测CA242在恶性肿瘤诊断中的价值

Systematic analysis of CA242 within serum multiple tumor markers chip and its clinical significance in cancer diagnosis

目的探讨多肿瘤标志物蛋白芯片中糖链抗原CA242在各组人群中的表达、与其它检测指标的伴随情况及其临床意义。方法收集该院2004年～2011年可供分析的25 076例多肿瘤蛋白芯片检测结果,分析CA242在各组人群、各种肿瘤中的升高特点,CA242伴随其它检测指标升高情况以及在肿瘤中的分布情况。结果 CA242升高在恶性肿瘤患者中明显高于良性病变患者及正常体检者,而且以胰腺癌最高(56.02%),其次为肠癌(26.57%)、胃癌(23.47%);CA242伴随升高指标以CA19-9最为常见,其次为CEA、CA125、CA153,CA242/CA19-9伴随最常见于胰腺癌(100%),CA242/CEA伴随最常见于结直肠癌(90.73%)和肺癌(80.61%),CA242/CA125伴随最常见于肺癌(75.92%)和肝癌(71.43%);CA242+CA19-9+CEA联合升高最常见于结直肠癌,CA242+CA19-9+CEA+CA125联合升高最常见于肺癌。结论 CA242单项指标升高对胰腺癌、结直肠癌和胃癌的诊断具有较高价值,CA242伴随或联合CA19-9、CEA、CA125有利于提高胰腺癌、结直肠癌及肺癌诊断阳性率。

[ Objective ] To investigate the elevation of CA242 in various people and different tumor, as well as its situations along with other item of multiple tumor marker protein chip. [Methods] 25,076 cases detected with multiple tumor marker protein chip from 2004-2011 were analyzed by, in terms of the expression of CA242 in malignant tumor and heahhy people, and distribution combined with other items of multiple tumor marker protein chip by retrospective study. [ Results ] Elevated CA242 in patients with malignant tumors was significantly higher than in benign lesions and healthy people. The CA242 positive rate was the highest in pancreatic cancer （56.02%）, followed by colorectal cancer （26.57%） and gastric cancer （23.47%）. The commonest accompanied item is CA19-9, followed by CEA, CA125 and CA153. The CA242/CA19-9 was most observed in pancreatic cancer （100%）, CA242/ CEA most observed in colorectal cancer （90.73%） and lung cancer （80.61%）, CA242/CA125 most observed in lung cancer （75.92%） and hepatocellular carcinoma （71.43%）. Moreover the CA242＋CA19-9＋CEA combined pattern was commonest in colorectal cancer, CA242＋CA19-9＋CEA＋CA125 combined pattern was commonest in lung cancer. [Conclusion] The positive of CA242 in pancreatic cancer, colorectal cancer and gastric cancer has a high diagnostic value. Combined detection of CA242 and CA19-9, CEA or CA125 is clinically useful for early pancreatic cancer, colorectal cancer and lung cancer.
