大鼠严重烧伤后核黄素早期使用对肠源性感染的影响　

An experimental study on the effect of early use of riboflavin on enterogenous infection after thermal injury in rats

目的 观察早期使用核黄素对烧伤后肠源性感染发生、发展的影响。方法采用大鼠３０％Ⅲ度烧伤模型并通过胃肠道管饲产灵菌红素的粘质沙雷氏菌示踪，检测血浆内毒素含量和肠道示踪菌的播散情况，比较核黄素腹腔治疗与单纯烧伤之间的差异。结果治疗组伤后１２ｈ血浆内毒素含量显著低于烧伤对照组（Ｐ＜０．０１），伤后２４ｈ治疗组肠道示踪菌较对照组在血液、痂下、肾、肺的阳性检出率明显降低（Ｐ＜０．０５）：肺、肾、痂下等组织的示踪菌定量也明显降低（Ｐ＜０．０５）。结论 核黄素的早期使用可降低严重烧伤大鼠肠源性内毒素的吸收入血量，减轻肠道细菌的全身播散发生率及其程度。

Objective To observe the effect of early use of riboflavin on the enterogenous infection following severe bums in rats. Methods Sixty Wistar rats were divided into 3 groups; treated group (20 rats), control group (20 rats), and normal group (20 rats). A model of 30% TBSA full thickness scald was created on the back of rats in the treated and control group, and serratia macescens producing prodigiosin were simulatneously introduced into the gastrointestinal tract in the 3 groups. Scalded rats were treated with riboflavin via intraperitoneal injection in treated group and the same volume of saline in control group. The concentration of endotoxin in plasma and the tracing bacteria migrating to varied tissues were determined. Results in treated group, the level of endotoxin in plasma decreased significantly 12 h post-burn (P<0.01 ) and the positive rate of tracing baCteria migrating into blood, subeschar tissue, kindneys and lunar was reduced as well (P<0.05 ), and the quantity of above microbe in kidneys, lungs and subeschar tissue was lowered (P<0.05 ) compared with that of control group. Conclusion The early use of riboflavin after bums could improve the condition of endotoxemia and alleviate the extensive migration of gut flora.
