目的 探讨宫腔内人工授精（ＩＵＩ）的最佳时机以提高妊娠率。对象 ４０例５３个周期男性不孕及不明原因不孕症患者，分为三组：自然周期组９例１４个周期，用克罗米芬（ＣＣ）促排卵组１５例１７个周期，ＣＣ＋ＨＭＧ＋ＨＣＧ促排卵组１６例２２个周期。方法经阴道Ｂ超监测卵泡发育并测量子宫内膜厚度及超声类型，自主卵泡平均直径达１．６ｃｍ以上时开始进行ＬＨ测定。结果 （１）自然周期于卵泡平均直径达１．

Objective:Searching for the optimal time of IUI to increase the pregnancy rate.Methord:53 cycles in 40 couples were divided into three groups:(1)natural cycle group(14 cycles,9 patients).(2)clomiphene(C.C.)regimen group(17 cycles,15 patients).(3)C.C./HMG/HCG regimen(22 cycles,16 patients).Follicles,endometrial thickness and echopatterns were measured by transvaginal ultrasound.LH emerged was tested when the leading follicle reached 1 6cm.Results:1 71 42% natural cycles ovulated whithin 24 hours while 64 71% C.C.regimen and 68 18%.C.C./HMG/HCG regimen cycles ovulated 24 48 hours after the leading follicle reached 1 8cm.On the day prior to ovulation,the mean diameter of follicles were 1 94±0 11(cm)in natural cycles,2 26±0 26(cm)in C.C.regimen and 2 18±0 21(cm)in C.C./HMG/HCG regimen.2 Ovulation occured in 24 hours after LH emerged in natural cycles but 24 48 hours in ovulation induction cycles.Conclusion:The optimal time for IUI is as following:1 The thickness of endometrium greater than 9 mm in both natural cycles and ovulation induction cycles.2 The diameter of leading follicle reached 1 8cm in natural cycles and 2 1 2 2cm in ovulation induction cycles.3 Once LH surge,IUI should be performed within 24 hours in natural cycles but 24 48 hours in ovulation induction cycles.4 After ovulation,within 24 hours is another good time for IUI
