Detection of CK19 mRNA expression for diagnosis of lymph nodes micrometastasis in younger colorectal cancer patients by reverse transcriptase polymerase chain reaction

摘要 目的利用RT-PCR检测CK19 mRNA技术,从分子水平探讨在青年结直肠癌患者区域淋巴结微转移的价值及临床意义。方法采用RT-PCR扩增,对角蛋白CK19检测。对30例结直肠癌患者的癌组织标本和108个淋巴结进行检测,检测良性结直肠疾病(结直肠息肉、憩室等)手术的20例患者的正常肠周淋巴结20枚作为对照组,同时行常规病理检查。结果 30例结直肠癌患者均有CK19 mRNA表达,108个淋巴结中有47个存在CK19 mRNA阳性表达,阳性率为43.5%;常规病理检查有25枚有癌转移,阳性率为23.2%;20例良性病变患者30枚淋巴结CK19mRNA表达为阴性。结论 RT-PCR扩增CK19 mRNA检测结直肠癌区域淋巴结微转移,是一种敏感而特异性的检测方法,对青年结直肠癌患者指导治疗,有较大的临床意义。 Objective To evaluate the significance of cytokeratinl9 (CK19) expression of lymphy node of micrometastasis in youn- ger patients of colorectal cancer from molecular. Methods Using RT - PCR and routine histopathological examination we dectected the ex- pression of CKI9 mRNA in 108 lymph nodes from 30 younger patients with colorectal cancer and 20 lymph nodes from 20 patients with be- nign diseases( polyposis diverticulum) and speciens of 30 younger patients with colorectal cancer. Results CK19 mRNA expressed in all speciens of 30 younger patients with colorectal cancer. Among the 108 lymph nodes from 30 colorectal patients postive CK19mRNA expres- sion was detected in 47 lymph nodes (43.5%) ; Micrometastasis was detected in lymph nodes (23.2%) by routine histopathological exami- nation CK19 mRNA expression was not observed in all lymph nodes from the patients with colorectal benign diseases. Conclusion CKI9 mRNA detection by RT - PCR is superior to routine histopathological examination and is a highy sensitive and specific method, and it has importantly clinical significance in guiding treatment for younger patient of coloreetal cancer
