靶控输注依托咪酯在全身麻醉中的应用 被引量:6

摘要 目的探讨靶控输注依托咪酯在全麻中诱导及维持中的可行性。方法选择择期手术需插管全麻的患者48例。随机分为P组(丙泊酚组)、E组(依托咪酯组),每组24例。两组靶控输注丙泊酚或依托咪酯(效应室浓度分别为3mg/L及0.4mg/L),手术结束前5min停用丙泊酚或依托咪酯。记录两组麻醉前、诱导后1min、2min、插管即刻、插管后1min、3min及切皮时的SBP、DBP、HR,记录两组总麻醉时间、手术时间、患者清醒时间、拔管时间,患者术后恶心呕吐发生率;麻醉前、拔管后及术后第1天清晨抽静脉血2ml测血糖和皮质醇。结果 P组诱导后1min、2min SBP、DBP较气管插管前及E组明显下降(P<0.05),两组SBP、DBP、HR在气管插管后1min恢复到气管插管前水平;两组插管后皮质醇浓度均升高,且P组升高更显著(P<0.05),且在术后第1天清晨恢复到正常范围;两组围术期血糖均正常。两组总麻醉时间、手术时间、术后清醒时间、拔管时间、恶心呕吐发生率比较差异均无统计学意义。结论依托咪酯对血流动力学影响小,起效苏醒迅速,能更有效、更迅速地抑制围术期的各种应激反应,靶控输注可安全用于无内分泌疾病患者手术的麻醉。
