摘要
目的探讨食管癌和贲门癌根治切除术后并发吻合口瘘时临床治疗的方法及体会。方法选择从2008年8月到2011年5月在我院接受根治切除术治疗的379例食管癌和贲门癌患者中有16例患者手术后并发吻合口瘘,对这16例食管癌和贲门癌患者的临床资料进行回顾性分析。结果 16例患者中胸内吻合口瘘有12例,1例死亡;颈部吻合口瘘有4例,无患者死亡。结论根据手术后吻合口瘘出现的原因,在术中和术后加强管理积极预防术后吻合口瘘的发生;及时发现术后吻合口瘘并尽早给予积极正确处理可以降低吻合口瘘的死亡率。
Objective To investigate the radical resection of esophageal and gastric cardia postoperative anastomotic fistula treatment methods and clinical experience.Methods From August 2008 to May 2011 in our hospital for radical resection of 379 cases of esophageal and gastric cardia in patients after surgery,16 patients with anastomotic leakage,these 16 cases of esophageal and gastric cardia clinical data were retrospectively analyzed.Results 16 patients with intrathoracic anastomotic leakage in 12 cases,1 patient died;cervical anastomotic fistula in 4 patients,no patients died.Conclusion Based on post-operative anastomotic leakage occurs because,in strengthening the management of intraoperative and postoperative active prevention of postoperative anastomotic fistula;to detect early postoperative anastomotic leakage and proper treatment can give a positive lower mortality rate of anastomotic leakage.
