胸腔闭式引流并胸腔灌注甘露聚糖肽联合顺铂治疗恶性胸腔积液50例疗效观察

CLINICAL COMPARATIVE ANALYSIS OF THORACIC CLOSE DRAINAGE WITH MANNATIDE INTRACAVITARY ADMINISTRATION IN COMBINATION WITH CISPLATIN IN THE TREATMENT OF MALIGNANT PLEURAL EFFUSION

目的对比观察中心静脉导管胸腔闭式引流并胸腔灌注甘露聚糖肽联合顺铂及单用顺铂治疗恶性胸腔积液的临床疗效和毒副反应。方法 96例恶性胸腔积液患者随机分为二组:观察组50例,对照组46例,行中心静脉导管胸腔闭式引流术,待胸腔积液排尽后,观察组胸腔灌注甘露聚糖肽和顺铂,对照组胸腔灌注顺铂,二组均每周1次,连续治疗2～4周。结果观察组有效率76.0%,对照组有效率52.2%,观察组优于对照组(P<0.05);观察组生活质量改善率78.0%,对照组改善率54.35%,观察组优于对照组(P<0.05);二组毒副反应比较差异无统计学意义(P>0.05)。结论经中心静脉导管胸腔闭式引流并胸腔灌注甘露聚糖肽联合顺铂治疗恶性胸腔积液临床疗效好,毒副反应轻,能够提高患者生活质量。

Objective To investigate the efficacy,toxicity and side effect of thoracic close drainage via central venous catheter filled with mannatide by intracavitary administration in combination with cisplatin alone in the treatment of malignant pleural effusion.Methods 96 patients with malignant pleural effusion were randomly divided into 2 groups,50 patients in the treatment group and 46 patients in the control group.All patients underwent thoracic close drainage via central venous catheter,mannatide and cisplatin were injected into the thoracic cavity of the treatment group,while cisplatin was injected into that of the controll group.Treatment for both groups were repeated once per week and continued for 2~4 weeks.Results The response rate（RR）was 76% for the treatment group and 52.2℅ for the control group.The difference was statistically significant（P0.05）.Life quality improvement rate was 78% for the treatment group and 54.35% for the control group.The difference was statistically significant（P0.05）.The difference was not statistically significant between the toxicity and side effect in both groups（P0.05）.Conclusion The treatment of thoracic close drainage via central venous catheter with mannatide intracavitary administration in combination with cisplatin was effective and benefit to improve the patients＇s life quality,as well,no more incidence of toxicity or side effect in the patients with malignant pleural effusion.
