动力髋螺钉和人工股骨头置换治疗80岁以上骨质疏松性粗隆间骨折的疗效分析

Analysis of the treatment of osteoporotic femoral intertrochanteric fractures in senile with dynamic hip screw and artificial joints replacement

目的分析比较动力髋螺钉(DHS)和人工股骨头置换治疗80岁以上骨质疏松性股骨粗隆间骨折的临床疗效。方法回顾性分析我院2000年1月～2010年1月80岁以上骨质疏松性股骨粗隆间骨折人工股骨头置换和DHS的治疗效果(人工股骨头置换组35例和DHS组38例),比较各组手术时间、术中平均出血量、围手术期并发症及Harris髋关节功能评分等指标。结果人工股骨头置换组较DHS组手术时间长、出血量多,但下床时间早,术后功能恢复好。结论与DHS相比较,人工股骨头置换更适合治疗80岁以上骨质疏松性粗隆间骨折。

Objective To compare the clinical effects of artificial joints replacement and dynamic hip screw in the treatment of intertrochanteric fractures in elderly patients with osteoporosis.Methods Retrospective analysis was performed in 73 cases of senile intertrochanteric fractures from Jan.2000 to Jan.2010.Totally 35 received artificial joints replacement with a prosthesis specially designed for intertrochanteric fractures,and 38 underwent dynamic hip screw fixation.The operation time,mean intraoperative blood loss,postoperative complications,and the Harris hip function scores were observed.Results The artificial joints replacement group had significantly longer operation time,more intraoperative blood loss,but had shorter time to begin weight-bearing activity and higher postoperative Harris hip score.Conclusion Artificial joints replacement is preferable to dynamic hip screw in elderly patients with osteoporosis.
