摘要
目的探讨甲胎蛋白(AFP)、人肝癌抗原(PHCA)和糖链抗原19-9(CA19-9)联合检测对原发性肝癌(HCC)的诊断价值。方法对39例HCC(HCC组)和36例肝硬化患者(肝硬化组),以及40例健康体检者(正常对照组),进行AFP、PHCA和CA19-9三指标检测,分析3项指标对HCC的诊断价值。结果 HCC组血清AFP、PHCA、CA19-9的水平明显高于肝硬化组(P<0.05);三者曲线下面积分别为0.872、0.839、0.649;PHCA的临界值为25.79U/L;HCC组中AFP、PHCA、CA19-9联合检测的诊断敏感性为92.3%,与单项指标的敏感度71.8%、71.8%、61.5%相比,显著增高(P<0.05)。结论 AFP、PHCA、CA19-9三指标联合应用,提高HCC的诊断灵敏度,提高诊断水平。
Objective To investigate the application value of combined detection of alpha-fetoprotein ( AFP), human hepatocellular carcinoma antigen( PHCA), and carbohydrate antigen 19-9 (CA19-9)in serum in the diagnosis of hepatocellular carcinoma. Methods The three indexes including AFP, PHCA and CA19-9 were detected in 38 cases of HCC( HCC group) ,36 cases of cirrhosis( cirrhosis group), and 40 healthy subjects( normal control group) ,and the diagnostic value of combined detection for HCC was evaluated. Results The serum levels of AFP, PHCA and CA19-9 in HCC group were significantly higher than those in cirrhosis group( P 〈 0.05). The area under the curve of the three indexes was O. 872,0. 839,0.649, respectively, and the critical value of PHCA was 25.79U/L according to ROC curve. In the HCC group ,the diagnostic sensitivity of combined detection of AFP, PHCA and CA19-9 was 92.3% ,which was significantly higher than that of single detection of AFP(71.8% ), PHCA(71.8%)or CA19-9(61.5%) ( P 〈0.05). Conclusion The combined detection of AFP, PHCA and CA19-9 can enhance the diagnostic sensitivity and improve diagnostic level for HCC.
