AO锁骨钩钢板治疗Ⅲ度肩锁关节脱位44例临床分析

CLINICAL ANALYSIS OF 44 CASES Ⅲ DEGREE ACROMIOCLAVICULAR JOINT DISLOCATION TREATED WITH AO CLAVICULAR HOOK PLATE

目的总结AO锁骨钩钢板治疗Ⅲ度肩锁关节脱位的临床经验。方法回顾性分析44例Ⅲ度肩锁关节脱位病人进行AO锁骨钩钢板治疗的临床资料。结果本组病例中优41例,良2例,差1例。优41例术后病例肩关节形状正常,无异常隆起,按压锁骨远端按琴征消失,肩关节屈伸、外展、内收等活动均正常,恢复正常的生活和工作。良2例出现肩峰下撞击征,差1例为取出内固定后再脱位,重新行手术治疗并修补喙锁韧带,术后16个月拆除内固定后脱位无再发。结论该手术简单易行、创伤少、固定可靠、疗效好、不损伤关节面、对肩关节早期功能锻炼有帮助,是治疗Ⅲ度肩锁关节脱位的有效方法。

Objective To summary the experience in AO clavicular hook plate treatment for Ⅲ degree acromioclavicular joint dislocation. Methods Retrospectively analyze the clinical data of 44 cases Ⅲ degree acromioclavicular joint dislocation treated with AO clavicular hook plate. Results The outcome of this group of cases,41 cases were excellent,good in 2 cases,poor in 1 case.In excellent 41 cases the postoperative shoulder joint shape were normal,no abnormal uplift,pressing the distal clavicle the piano syndrome disappeared,shoulder flexion,abduction,adduction,extension activities were normal,the patient get normal life and work.Good in 2 patients with subacromial impingement syndrome,1 case dislocation recurrent after removal of implants,had to reoperation and repair of coracoclavicular ligament,16 months after the removal of internal fixation after dislocation without recurrence. Conclusion This operation is simple,less trauma,reliable fixation,good curative effect,no damage to the articular surface,helpful to the shoulder joint early functional exercise,is the effective treatment of acromioclavicular joint dislocation.
