紫外线照射充氧自血回输治疗老年胃十二指肠溃疡临床观察

Clinical study of ultraviolet blood irradiation and oxygenation on peptic ulcer in senile persons

目的 观察紫外线照射充氧自血回输（ＵＢＩＯ）治疗老年消化性溃疡的疗效和机制。方法 将６０ 例老年消化性溃疡患者随机分成二组，３０ 例采用ＵＢＩＯ 疗法并用雷尼替丁，另３０ 例作为对照组仅服雷尼替丁。ＵＢＩＯ每周３ 次，４ 周１ 疗程。结果 ＵＢＩＯ组和对照组溃疡愈合率分别为９０ ％（２７／３０）和６７％ （２０／３０），χ２ ＝ ４ ．８１ ，Ｐ＜ ０．０５，差异有显著性；幽门螺杆菌（ＨＰ） 阴转率分别为５８ ％（１４／２４）和９ ％ （２／２３），χ２ ＝ １２．８９ ，Ｐ＜ ０ ．０１，差异有非常显著性；ＵＢＩＯ 组疗程前后纤维蛋白原、全血粘度、红细胞聚集指数等方面改善非常显著（ Ｐ＜ ０ ．０５ ～０ ．０１） ，血浆铜锌超氧化物歧化酶（ＳＯＤ－ １）明显升高。溃疡愈合患者随访１ 年，溃疡复发率ＵＢＩＯ 组为２１％ （５／２４） ，对照组为５６％ （１０／１８） ，χ２＝５ ．４０ ，Ｐ＜ ０ ．０５ ，差异有显著性。结论 ＵＢＩＯ 能明显降低血液粘稠度，杀灭ＨＰ，提高ＳＯＤ－ １ 含量，它既能清除攻击因子又能增强防御因子，明显地提高老年消化性溃疡的愈合率，降低复发率。

Objective The purpose of this paper was to study the effect and mechanism of ultraviolet blood irradiation and oxygenation(UBIO) on paptic ulcer of senile persons.Methods Sixty senile patients with peptic ulcer were randomly allocated into two groups: Thirty cases were received the treatment of UBIO combined with ranitidine(UBIO group);the UBIO therapy was 3 times/per weeks for 4 weeks.Other 30 cases as a control group were only treated with ranitidine.Results The ulcer healing rate and clearance rate of Helicobacter Pylori(HP) in UBIO group was respectively 90% and 58% compared with to 67% and 9% of control group.There was a significant difference in both groups(P<0.01).At pre- and post-UBIO treatment, there were obvious differences in fibrinogen,whole blood viscosity,plasma viscosity and RBC aggregation index.The value of SOD-1 was also raised apparantly.Through followed-up of 12 months ,the ulcer recurrence rate of UBIO and control group was separately 21% and 56%(P<0.05).Conclusion It is demonstrated that UBIO may reduce the blood viscosity, kill HP and increase the value of SOD-1. It may get rid of some attack factors and improve mucosal barrier,evidently increase the ulcer healing rate and decrease the ulcer recurrence rate.
