细胞学检查在基层医院肺癌诊断中的应用

Study on cytological diagnostic accuracy of lung cancer in primary hospital

目的探讨痰液细胞学与针吸细胞学在基层医院诊断肺癌中的应用价值。方法 1)取痰液有效部位制作涂片,行细胞学检查;2)影像引导下经皮行肺部病变细针穿刺,针吸物涂片细胞学检查。结果痰细胞学检查阳性率62.2%,分型符合率72.0%;肺针吸细胞学检查阳性率83.7%,分型符合率86.3%;两者结合,阳性率95.8%,高于痰液细胞学组和肺针吸细胞学组。结论痰液细胞学与肺针吸细胞学检查诊断率均令人满意;肺针吸细胞学检查适用于各部位肺癌,尤以周围型更佳;两者结合具有很高的应用价值。

Objective To evaluate the application significance of sputum cytology and FNA in the diagnosis of lung cancer. Methods 1） The sputum smear was made by effective portion of the sputum for cytological diagnosis. 2） Imaging guided fine needle aspiration biopsy was performed in lung lesion, all the samples were prepared to make smeare. Results The positive rate of sputum cytology was 62. 2% and the coincidence rate with pathological type was 72.0%. The positive rate of FNA was 83.7%, and the coincidence rate with pathological type was 86.3%. The positive rate was significantly higher combining with two methods（95.8%）. Conclusions The positive rate of sputum cytology or FNA was satisfied. FNA could be applied in all types of lung cancer, especially for the peripheral type lung cancer. Combing with the two methods has highly clinical value.
