摘要
目的探讨AO锁定加压板(LCP)加植骨内固定治疗尺桡骨骨不连的疗效。方法2001年3月-2010年11月对26例尺桡骨骨不连行手术内固定,训练摔伤12例、机器绞伤4例、砸伤6例、交通伤4例。开放性骨折9例,闭合性骨折17例。其中尺桡骨不愈合5例,尺骨不愈合13例,桡骨不愈合8例。全部采用LCP内固定加自身髂骨植骨。结果26例经4-12个月,平均6个月的随访。骨不连全部愈合,无钢板螺钉断裂、松动、疲劳骨折及再骨折等并发症,疗效满意。结论采用LCP内固定加植骨治疗尺桡骨骨不连符合生物学固定原则,具有操作简便,可提供坚强、有效的内固定,有利于前臂、关节早期功能锻炼,减少了骨不连的发生,是治疗尺桡骨骨不连的有效方法之一。
[ Objective ] To evaluate the effectiveness of AO locking compression plate (LCP) plus bone graft internal fixation in treating Ulna and Radius nonunion. [Methods] From March 2001 to November 2010, there were 26 cases of internal fixation operations for Ulna and Radius nonunion, 12 cases of which were caused by training injury, 4 cases by mechanical twist, 6 cases by crashing injury, and 4 cases by traffic injury. 9 cases were open fracture, while 17 cases were closed fracture. Among which, 5 cases were Ulna and Radius ununited fracture, 17 cases were Ulna ununited fracture, 8 cases were Radius uuunited fracture. LCP internal fixation plus self iliae bone transplantation were employed in all these cases above. [Results] After the follow-up visits ranged from 4 to 12 months, 6 months in average, all the nonunions were unioned. It was satisfied that no complications of screw fracture and loosening in steel plate, stress fractures and re-fractures were observed. [ Conclusion] According with the biological fixation principle, the employment of LCP internal fixation plus bone graft in treating Ulna and Radius nonunion characterized in the convenient operation, which could afford strong and effective internal fixation, be useful to the functional exercises of forearms and joints in early stage and reduce the occurrence of nonunion. It became a valid method for Ulna and Radius nonunion treatment.
