摘要
目的探讨新诊断的T2DM患者高血糖状态纠正前后血清铁蛋白(SF)的变化。方法对120例新诊断的、FPG≥10mmol/L的T2DM患者行胰岛素强化治疗,观察高血糖状态纠正前后SF及多项临床资料,并与正常对照组资料进行比较。结果与对照组比:T2DM患者治疗前存在较高的C-RP、FPG、2hPG、HbA1c、SF、Fe、TS、Hb、RBC、MCV、MCH和MCHC,较低的C-P曲线下面积和P曲线下面积和TIBC,差异有统计学意义(P<0.05);治疗后与治疗前比:BMI、WHR、SBP、DBP、TC、TG、HDL-C、LDL-C、Fe、TS、Hb、RBC、MCV、MCH、MCHC和TIBC浓度差异无统计学意义(P>0.05),C-RP、SF明显降低,但仍高于对照组,C-P曲线下面积明显升高,但仍低于对照组,差异有统计学意义(P<0.01)。结论胰岛素强化治疗纠正高血糖状态能够使血清铁蛋白浓度降低。
Objective To investigate the changes of the serum ferritin(SF) before and after controlling the hyperglycosemic state in newly diagnosed type 2 diabetes mellitus(T2DM) patients. Methods 120 patients with newly diagnosed type 2 diabetes mellitus (fasting plasma glucoseS10 retool/L) received intensive insulin therapy, whose SF and clinical data were measured before and after controlling the hyperglycemia state and were compared to normal control group. Results The serum hsCRP, FPG, 2hPG, HbA1c, SF, Fe, TS, Hb, RBC, MCV, MCH and MCHC were higher in T2DM than in control group(P〈0. 05-0. 01), area under C-peptide curve and TIBC were lower in T2DM than in control group (P〈 0. 01). After controlling the hyperglycemia state, there was no difference in BMI, WHR, SBP, DBP, TC, TG, HDL-C, LDL-C, Fe, TS, Hb, RBC, MCV, MCH, MCHC and TIBC between pre- and post-therapy(all P〉0. 05). C-RP and SF levels were lower in T2DM(P〈0. 01), but still significantly higher than that in control group (P〈O. 01). Conclusion Intensive insulin theraov can partly reduce SF level.
