3.0T动态增强磁共振淋巴造影对下肢淋巴水肿淋巴管功能的评估价值 被引量:6

Functional Evalution of Lymphatic Vessels of Low Extremity Lymphedema with 3.0 T Dynamic Enhanced MR Lymphangiography

摘要 目的探讨3.0 T动态增强磁共振淋巴造影在评估下肢淋巴水肿淋巴管形态及功能的应用价值。资料与方法使用Philips Achicva 3.0 T高场强超导型MR机,对25例下肢淋巴水肿患者(34侧患肢)行三维动态增强MR淋巴造影检查,观察并记录淋巴管形态改变,测量对比剂注射后5、10、15、20、25、30 min淋巴管感兴趣区相对信号强度(RSI),绘制淋巴管时间-信号强度曲线(TIC),计算峰值时间及洗净率。结果 34侧不同分期的患肢中,动态增强MR淋巴造影能显示淋巴管形态改变、确定淋巴管阻塞部位及反映淋巴管病理生理过程;淋巴管呈延迟强化,时间均超过30 min,且不同分期淋巴管强化的峰值时间、洗净率及各时间点的RSI差异有统计学意义。结论动态增强磁共振淋巴造影能为不同分期的下肢淋巴水肿患者提供淋巴管形态和功能信息,是一种安全可行的新技术。 Objective To discuss the value of 3.0T dynamic enhanced MR lymphangiography in evaluating the configuration and function of lymphatic vessels in low extremity lymphedema.Materials and Methods 25 patients with low extremity lymphedema(34 affected low extremity) received dynamic enhanced MRL,the morphologic changes of the lymphatic vessel were observed and recorded,and the relative signal intensity(RSI) curve with the time point of 5 min,10 min,15 min,20 min,25 min and 30 min after contrast injection,time signal intensity curve(TIC),T peak and wash in rate were then measured.Results The configuration changes,the obstructed position and the pathophysiological processes of lymphatic vessel could be demonstrated by DEC MRI.lymphatic vessels could be displayed at delayed stage with the continuing time over 30 mins.There were significant differences of T peak,wash in rate and RSI in different stage.Conclusion MR lymphangiography is a safe,feasible and new imaging method to provide more useful information of the configu ration and function of lymphatic vessels in patients with lymphedema.
