右美托咪定复合丙泊酚在老年食管异物取出术中的临床研究

目的:观察右美托咪定复合丙泊酚在老年食管异物取出术中的麻醉效果和安全性。方法:选择50例ASAⅠ～Ⅱ级因食道异物梗阻需要取出的老年患者,随机分为两组,每组25例,M组为右美托咪定复合丙泊酚组,P组为丙泊酚组。观察两组手术中在注药前(T0)、注药后2分钟(T1)、食道镜置入食道时(T2)、取异物时(T3)、术毕退出食道镜时(T4)、苏醒时(T5)六个时点患者的血压、心率、血氧饱和度的变化,以及麻醉镇痛效果、麻醉起效时间、苏醒时间、丙泊酚用量、患者不良反应等。结果:M组较P组循环波动少、呼吸抑制少、镇痛效果满意、苏醒时间快,丙泊酚用量少。患者不良反应少,差异有统计学意义(P<0.05)。结论:右美托咪定复合丙泊酚可以安全用于老年人食道异物取出且麻醉效果满意。

Objective： To investigate the effects of propofol combined with dexme- detomidine on extraction with foreign body in esophagus in elderly patients. Methods： Select 50 ASAⅠ-Ⅱ level because of the need to remove esophageal foreign body ob- struction elderly randomly divided into two groups,each set of 25 cases, the m - group for the dexmedetomidine combined with propofol groups, p - group is group of propo- fol. Observation two group surgery in the in note drug Qian （TO）, and note drug Hou 2min（T1 ）,and esophageal mirror reset into esophageal Shi （ T2 ） , and take foreign body Shi （T3）, and operation completed exit e- sophageal mirror Shi （ T4 ） , and wake up Shi （ T5 ） six a point patients of blood pressure, and heart rate, and blood oxygen saturated of of changes ; Anesthesia Analgesia effect ; anesthesia up effect time, and wake up time, and cparking phenol dosage;patients bad re- action. Results ：M group less than p- group cycle fluctuation,less respiratory depression and analgesia satisfactory, the wake up time fast, propofo] dosage less. Adverse reactions in patients with small differences are statisti- cally significant （ P 〈 0. 05 ）. Conclusion： The anesthetic effect of dexmedetomidine combined with propofol for elderly patients in extraction with foreign body in esophagus is satisfactory and safety.
