跌倒高危患者跟踪评估监控流程的设计和应用

Design and application of assessment system for monitoring high risk fall patients

目的探讨跌倒高危患者跟踪评估监控流程设计方法和应用效果。方法制订跌倒风险评估监控表和跌倒高危患者跟踪评估监控流程,将跌倒风险评估监控表和跌倒高危患者跟踪评估监控流程应用于临床并进行动态管理。观察实施前后患者跌倒的发生率。结果实施流程管理后,跌倒高危预报率增加,患者跌倒发生率降低,实施前后比较,P<0.05,差异具有统计学意义。结论实施跌倒高危患者跟踪评估监控流程管理能降低跌倒发生率。

Objective To investigate design and application of assessment system for monitoring high risk fall patients. Method An assessment system for monitoring high risk fall patients was formulated so that the fall risk assessment scale and the assessment system were used for dynamic management clinically. Result The prediction rate for high risk falls was significantly increased and the fall rate of the hospitalized patients was significantly decreased, compared to the rates before the use of the assessment system （P 0.05）. Conclusion The use of assessment system for monitoring high risk fall patients may prevent the falls of high-risk fall patients and reduce the fall rate.
