应用音乐干预对提高心脏介入术中患者舒适度的效果研究

Effects of music intervention on improving the degree of comfort of patients during cardiac intervention surgery

目的探讨音乐干预对提高心脏介入术中患者舒适度的临床应用效果。方法选择行择期心脏介入手术患者80例作为研究对象。按手术时间顺序随机分为对照组和实验组各40例。在手术期间，对照组按常规进行术中护理，实验组在常规护理的同时给予患者双耳戴耳挂式耳机连接MP3欣赏音乐至手术结束。同时采用焦虑自评量表（SAS）和文字描述评分法（VDS）分别评定两组患者术前，术中心理状态及腰背疼痛情况，观察记录患者有无肌紧张、入睡困难。结果两组患者在术前1d开始出现不同程度焦虑和不舒适反应，SAS、VDS评分及不舒适反应例数两组间比较差异均无统计学意义P〉0．05；手术期间，SAS、VDS评分及不舒适反应进行组间比较，实验组明显低于对照组，差异均有统计学意义P〈0．05。结论在心脏介入术中应用耳挂式音乐干预方法，可以去除术中医源性噪音的不良刺激，使患者在欣赏音乐的同时分散对手术过程的注意力而抒缓其焦虑心理和不适感，达到提高术中患者舒适度的目的。

Objective To study the clinical effects of music intervention on improving the degree of comfort of patients during cardiac intervention surgery. Methods A total of 80 patients who were going to undergo elective cardiac intervention were randomly divided into control group （40cases） and （40cases） according to operation time sequence. During cardiac intervention surgery, the patients in control group received routine care and the patients in experimental group received music intervention through supra - aural earphone connected to MP3 on the basis of routine care. At the same time, the scales of SAS and VDS were applied to evaluate the mental state and lumbar and back pain of patients before and during operation. And muscle tonus or difficulty falling asleep were observed and recorded. Results Pa- tients became anxious and uncomfortable 1 clay before operation and there was no statistic significant difference in the score of SAS and VDS or the cases of uncomfortable reaction between two groups （P 〉0. 05）. But during operation, the differences were statistic significant （P 〈 0. 05）. The score of SAS, VDS and the uncomfortable response were obviously lower in experimental group compared with control group. Conclusions Music intervention in cardiac intervention surgery can degrade the infaust stimulus of iatrogenic noise and distract the patients ＂attention when they are enjoying the music during operation, then ease their anxiety and discomfort, at last raise the degree of comfort.
