BAEP预测自发性蛛网膜下隙出血昏迷病人预后价值

THE VALUE OF BRAIN STEM AUDITORY EVOKED POTENTIAL IN PREDICTING THE PROGNOSIS OF COMATOSE PATIENTS DUE TO SPONTANEOUS SUBARACHNOID HEMORRHAGE

摘要 目的探讨脑干听觉诱发电位(BAEP)监测对自发性蛛网膜下隙出血导致昏迷病人预后的预测价值。方法对2010年1—12月我科收治的42例自发性蛛网膜下隙出血昏迷病人,进行格拉斯哥昏迷评分(GCS)和BAEP检查,并计算两种方法的预后预测准确率。结果 GCS评分预测准确率为73.8%,而BAEP预测准确率为90.5%,两者比较差异有显著性(χ2=3.977,P<0.05)。6例GCS评分低,同时BAEP重度异常病人,最后均恢复不良。结论 BAEP能更敏感地反映昏迷病人脑功能状态;多种检查结合,可以使预测更为准确。 Objective To assess the value of brain stem auditory evoked potential(BEAP) in predicting the prognosis of comatose patients caused by spontaneous subarachnoid hemorrhage(SSH).Methods From January to December of 2010,42 comatose patients due to SSH were treated at our department.Glasgow coma scale(GCS) and BAEP inspection were done,and their accuracy for predicting the patients' prognosis was compared.Results The accuracy of prediction was 73.8% for GCS and 90.5% for BEAP,the difference was significant(χ2=3.977,P0.05).A poor prognosis was noted in six patients with low GCS score and severely abnormal BAEP.Conclusion BEAP can more sensitively reflect the brain function of comatose patients.The prediction would be more accurate by using a combination of various examinations.
