Medical Cost of Burned Children and Its Influencing Factors

目的了解烧伤患儿住院费用构成情况及其影响因素,探讨控制烧伤患儿住院医疗费用的有效方法。方法抽取烧伤科2010年1—12月137例住院烧伤患儿(14岁以下)的病例首页,对住院费用构成情况及影响因素进行分析。结果本组137例烧伤患儿住院费用中治疗费用比例最高,占47.8%;其次是药品费用,占33.9%;其余费用共占18.3%。治疗费用、药品费用、检查费用、浅Ⅱ°烧伤面积、手术与否、住院时间、深Ⅱ°烧伤面积、烧伤总面积、Ⅲ°烧伤面积是影响烧伤患儿住院费用的主要因素。结论降低烧伤患儿住院费用的关键是积极处理深度烧伤,以减少住院时间,同时加强医院标准化管理,减少不必要的临床用药,采取综合措施切实降低烧伤患儿住院费用。

Objective To understand the composition of medical cost of burned children and its influencing factors and to explore an effective method to reduce the medical cost. Methods Totally, 137 pieces of medical records of burned children younger than 14 years were analyzed for the composition of medical cost and its influencing factors. Results Treatment costs, accounting for 47.8%, was on the top of medical cost, followed by medicine costs, accounting for 33.9%, and other costs were accounted for about 18.3%. Treatment costs, medicine costs, depth of the burn area, total burn area, surgery, length of hospital stay were main factors which influenced medical expenses of children patients. Conclusion The key measure to reduce the total hospital costs of burned children is the positive treatment of deep burn, shortening the hospital stay, and standardized management should also be strengthened so as to reduce unnecessary clinical medicine. Comprehensive measures could contribute to the reduce of the medical cost.
