摘要
目的探讨MRI在脑静脉窦血栓(cerebral venous sinus thrombosis,CVST)合并脑实质损害的诊断和评价中的作用。方法回顾性分析北京大学深圳医院2003年5月~2010年8月期间收治的38例CVST患者的相关临床资料及影像学资料。全部病例经临床资料及随访证实,所有病例均作常规MRI的T_1WI、T_2WI序列及MRV,MRV采用三维对比增强法,分析、探讨脑静脉窦血栓合并脑实质损害的MRI表现。结果本组38例经MRI及MRV检查均提示颅内静脉窦血栓存在,14例患者并发脑实质损害,其中脑梗塞9例,脑出血5例。结论 MRI可显示脑静脉窦血栓直接征象,同时能较好的显示脑梗塞、脑出血、脑水肿等并发症,综合应用常规T_1WI、T_2WI序列及MRV可无创诊断CVST,同时为CVST的病情评估提供重要信息。
Objective To assess the value of MRI for detecting cerebral venous sinus thrombosis (CVST) and complicated brain injury. Methods Thirty-eight patients with CVST confirmed by clinical follow-up and imaging from May 2003 to August 2010 were included in the retrospective analysis. Conventional MRI, magnetic resonance venography (MRV) ,three-dimensional contrast-enhanced MR angiography (3D-CE MRA) were reviewed for the presence of CVST and secondary brain parenchymal damages. Results All of the 38 patients were correctly diagnosed as CVST on MRI. 14 patients suffered complications of cerebral infarction (9) or cerebral hemorrhage (5). Conclusion Conventional MRI and MRV Allnw accurate diagnosis of CVST and complications including cerebral infarction, hemorrhage and cerebral edema.
