累及野照射在调强放疗非小细胞肺癌中的应用

Application of involved-field radiation in intensity modulated radiation therapy for non-small cell lung cancer

目的探讨累及野照射在调强放射治疗非小细胞肺癌(NSCLC)的临床价值。方法63例NSCLC患者随机分为选择性淋巴引流区照射组(ENI组,33例)和累及野照射组(IFI组,30例),采用不同物理剂量学参数(TAD,CI,DHI)的调强放射治疗技术。结果 IFI组有效率明显高于ENI组(76.7%vs.48.5%)(P<0.05);两组治疗相关并发症的发生率和1、2年生存率相仿(P>0.05)。结论对于不能手术的NSCLC患者,采用累及野照射的调强放射治疗可以提高近期疗效。

Objective To evaluate the effect of involved-field radiation（IFI） in intensity modulated radiation therapy（IMRT） for non-small cell lung cancer（NSCLC）. Methods A total of 63 patients with inoperable NSCLC was randomly divided into 2 groups of IFI（30 cases,with IFI） and ENI（33 cases,with elective nodal radiation）,who underwent IMRT with different physical dosages of TAD,CI and DHI. Results The effectiveness rate was higher in group IFI than that in group ENI（76.7% vs.48.5%）（P0.05）.The incidence of radiation-related complications and 1-,2-year survival rates of two groups were similar（P0.05）. Conclusion IMRT with IFI can improve the short-term efficacy for inoperable NSCLC.
