摘要
目的探讨嗜酸细胞性食管炎的诊断依据、特点,以降低嗜酸细胞性食管炎的误诊率。方法针对1例嗜酸细胞性食管炎的临床资料进行回顾性分析。结果嗜酸细胞性食管炎与食管癌误诊原因在于症状和影像学的相似性、无法活检、疾病地域高发食管癌等导致医师先入为主的诊断思维。结论嗜酸细胞性食管炎与食管癌的区别:①影像上食管黏膜是光滑的,包括钡餐和胃镜;②病情进展快,时发梗阻症状,处理后症状缓解明显,影像学进展较食管癌进展快;③病检区分明显。
Objective To investigate the diagnostic basis and characteristics of eosinophilic esophagitis, and to reduce the misdiagnosis rate of eosinophilic esophagitis. Methods Conducted a retrospective analysis of clinical data for one case of eosinophilic esophagitis. Results The causes of eosinophilic esophagitis misdiagnosed as esophageal cancer were the simi- larity of symptoms and imaging, could not biopsy, and high incidence of esophageal cancer, which leading to preconceived diagnostic thinking in physicians. Conclusion The difference between eosinophilic esophagitis and esophageal cancer： （1） esophageal mucosa is smooth on the image, including barium meal and gastroscopy; （2）it has rapid progression, and radio- graphic progression is quicker than esophageal cancer, obstructive symptoms occur from time to time and relieve after treatment; （3） pathological examination distinguishes significantly.
