新型防脱出螺钉治疗股骨颈骨折的生物力学特性

摘要 目的探讨自行设计的新型防脱出型多六角空心加压钛合金螺钉的生物力学特性。方法选择30对符合纳入标准(60个)福尔马林浸泡人尸体股骨标本分两组,即左右侧标本各一组,每组30个,一组用2枚新型防脱出型多六角空心加压钛合金螺钉固定,另一组用2枚AO空心加压螺钉固定;两组内标本对应成对(30对)随机分5个亚组,即A、B、C、D、E亚组,对应的亚组进行最大抗压强度、最大轴向拔出力、等量扭矩下的旋转角度、抗脱出力及螺钉进入关节腔最大压力测试并记录数值,并采用SPSS10.0统计软件分析。结果新型防脱出型多六角空心加压钛合金螺钉与AO空心加压螺钉在最大抗压强度和等量扭矩下的旋转角度上无统计学意义(P>0.05),新型防脱出型多六角空心加压钛合金螺钉最大抗脱出力、最大轴向拔出力、螺钉进入关节腔最大压力明显大于AO空心加压螺钉(P<0.05)。结论新型防脱出型多六角空心加压钛合金螺钉具有更好固定效果,同时能有效地防止螺钉脱出和螺钉穿出股骨头进入关节腔,也有利于取出。 Objective To evaluate biomechanical characteristics of Newly anti-extrusion multi-hexagonal cannulated titanium screw in the treatment for femoral neck fracture.Methods Thirty pairs samples(sixty samples) of femurs were divided into two groups:a group contained all right samples,the other one contained all left samples.Newly anti-extrusion multi-hexagonal cannulated titanium screw group and cannulated compression screws group;All samples were randomiy divided into five Sub-groups:A,B,C,D,E.each sub-group had twelve samples.To measure maximum resist compressive strength,maximum axial pull-out strength,rotation of same rotation torque,maximum pull-out strength of screw,the maximum strengths of screw camming into the joint of the corresponded subgroup.Results There was no statistics significance between the groups on the maximum resist compressive strength and rotation of same rotation torque;The remained characteristics that Newly anti-extrusion multi-hexagonal cannulated titanium screw has better than cannulated compression screws group.Conclusion Newly anti-extrusion multi-hexagonal cannulated titanium screw had better effect in femoral neck fracture,and had better effect in anti-pull-out strength and preventting screw caming into joint,and it was taked out.
