腹腔镜与开腹胃切除术治疗Ⅱ、Ⅲ期胃癌的对比研究

Comparative study of laparoscopic and open gastrectomy for gastric cancer(stage Ⅱ,Ⅲ)

目的:对比腹腔镜与开腹胃切除术治疗Ⅱ、Ⅲ期胃癌的近期疗效,探讨腹腔镜胃切除术治疗Ⅱ、Ⅲ期胃癌的可行性及安全性。方法:回顾分析2009年1月至2011年1月收治的74例Ⅱ、Ⅲ期胃癌患者的临床资料,其中38例行腹腔镜手术(腹腔镜组),36例行开腹手术(开腹组)。结果:两组患者术后并发症、术后胃肠功能恢复时间、围手术期死亡率、淋巴结数量、切缘阳性率均无显著差异,切口长度、手术时间、术中出血量、术后发热时间差异均有统计学意义。结论:腹腔镜胃切除术治疗Ⅱ、Ⅲ期胃癌安全可行,近期疗效与开腹手术相当。

Objective：To compare short-term outcomes of laparoscopic and open gastrectomy for gastric cancer（stage Ⅱ,Ⅲ）,and investigate the feasibility and safety of laparoscopy.Methods：Clinical data of 38 patients who underwent laparoscopic gastrectomy was compared with that of 36 patients who underwent open gastrectomy during Jan.2009 and Jan.2011.Results：There was no significant difference between the two groups in postoperative complications,bowel function recovery,perioperative mortality,number of lymph node harvested and positive margin.But the length of incision,blood loss,operative time and time of postoperative pyrexia were significantly different.Conclusions：Laparoscopic gastrectomy is a feasible and safe procedure for gastric cancer（stage Ⅱ,Ⅲ）,with similar short-term outcomes of laparotomy.
