股骨近端抗旋转髓内钉治疗股骨粗隆间骨折术中并发再骨折原因分析

An analysis on treating concurrent re-fracture of intertrochanteric fracture surgery with proximal femur anti-rotate intramedullary nail

目的:通过对2例股骨近端抗旋转髓内钉治疗股骨粗隆间骨折术中并发再骨折分析,探讨该手术注意事项及并发症防治。方法:对2例患者病史进行回顾性分析。结果:其中1例手术病人髓内钉直径过大,另1例髓内钉力线偏离股骨中心,术中进主钉困难,用力过大,最终术中并发骨折端再骨折。结论:股骨近端抗旋转髓内钉治疗股骨粗隆间骨折要注意术前对病人的评估,术中规范操作,维持解剖复位,选好进针点,避免医源性再骨折。

Objective： Concurrent re-fracture of intertrochanteric fracture surgery with proximal femur anti-rotate intramedullary nail was analyzed,discussing precautions in the Surgery and prevention of complications.Methods： Review the history of 2cases.Results： Intramedullary nail diameter in one case is too large,intramedullary nail lines of force in one case deviate from the femur center.Re-fracture fracture happened again in the surgery finally.Conclusion： Patients in the preoperative assessment,intraoperative regulate the operation,maintenance of anatomical reduction,selected the right points of the needle were needed to pay attention,to avoid iatrogenic re-fractures.
