CCP抗体、GPI抗原检测在类风湿关节炎中的诊断意义

Diagnostic value of detection of CCP antibody and GPI antigens in patients with rheumatoid arthritis

目的通过比较类风湿关节炎(RA)患者、非RA相关疾病患者抗环瓜氨酸肽抗体(CCP)、6-磷酸葡萄糖异构酶抗原(GPI)的表达及RA活动度临床指标的差别,比较两者在RA诊断中的意义。方法应用酶联免疫吸附试验(ELISA)对56例RA患者、30例非RA风湿病患者共86份血清标本进行检测。结果 RA患者GPI,抗CCP的阳性检出率分别为64.2%、85.7%,抗CCP联合GPI检测阳性率可提高到89.3%。抗CCP抗体阳性组与阴性组的关节肿胀数、关节压痛数、ESR差异有统计学意义(P<0.05);GPI阳性组与阴性组的关节肿胀数差异均有统计学意义(P<0.05),ESR差异无统计学意义(P>0.05)。结论联合检测CCP抗体、GPI抗原对诊断RA有意义并可能与RA的临床表现和活动性相关。

To compare the anticyclic citrullinated peptide antibodies （CCP） , 6 phosphate glucose isomerase antigen （GPI） expression and clinical parameters of RA activity difference in patients with rheumatoid arthritis（RA） and nonRA disease related. Methods 86 specimens were used for testing CCP and GPI in 56 patients with RA and 30 patients with non RA rheu matic diseases by using solid phase enzyme linked immunosorbent assay （ELISA）. Results GPI and anti CCP positive rates were respectively 64.2% and 85.7% in RA patients; GPI and anti CCP positive rate increased to 89.3%. Anti CCP antibody positive and negative groups in the number of joint swelling, joint tenderness, number and ESR was significant difference （ P 〈 0.05 ） ; GPI positive and negative groups in the number of swollen joints were significant differences （ P 〈 0.05 ）, ESR was no significant difference （ P 〉 0.05 ）. Conclusion Detection of CCP antibody and GPI antigen for diagnosis of RA may be associated with significant clinical manifestations of RA and activity and prognosis.
