摘要 目的:评价超声技术在检测高血压患者动脉缓冲功能中的应用价值。方法:对54例原发性高血压患者和10例正常人应用二维及多普勒超声分别对心脏和肱动脉扫查。肱动脉横断面顺应性(Cross-sectional Compliance CSC)和容积扩张性(VolumicDistensibility VD)作为反映动脉缓冲功能的指标。结果:正常对照组与高血压组相比,CSC、VD在高血压组显著升高,分别为 34.87±0.01 10^(-3)mm~2·mmHg^(-1)比 19.88±0.028 10^(-3)mm~2·mmHg^(-1),P=0.029;2.86±0.88 10^(-3)mm~2·mmHg^(-1) vs1. 61±0. 26 10^(-3)mm~2·mmHg,P=0. 0165。多因素分析结果显示收缩压,左室舒张早期和晚期峰值血流速度比(Pe/Pa),阻力指数(RI)是与CSC关系最为密切的参数。结论:超声具有直观、准确、实时反映动脉缓冲功能的特点,具有广阔的应用前景。 To determine clinical value of ultra- sound in evaluating arterial buffering function in hypertension. Methods: 2-DE and Doppler ultrasonography were applied to examine 54 essential hypertensive patients and 10 healthy subjects. Brachial artery cross-sectional compliance and volumic distensibility were determined that reflect arterial buffering function. Results: Brachial artery cross-sectional compliance, volumic distensibility were higher in hypertensive patients 34. 87± 0. 01 10^(-3) mm^2 .mmHg ^(-1) vs 19. 88±0. 028 10^(-3) mm^2 .mmHg^(-1),P = 0. 029; 2. 86 ± 0. 88 10^(-3)mmHg^(-1) vs 1. 61 ± 0. 26 10^(-3). mmHg^(-1), P= 0. 0165, respectively. Mutiple regression analysis results demonstrated systolic blood pressure,left ventricular diastolic peak early filling velocity and peak late filling velocity ratio, brachial artery blood flow resistant index had close relationship with brachial artery cross-sectional compliance. Conclusion: Ultrasound could examine arterial buffering function directly,accurately and real time.
