Analysis of left ventricular pressure during early relaxation in coronary artery disease

摘要 目的 探讨时间常数以评价冠心病舒张早期左室压力变化的意义。方法 冠状动脉(冠脉 )造影确诊正常及冠脉狭窄 >5 0 %共 6 0例 ;左心导管及左室电影造影测定血流动力学参数。结果 左室射血分数随着冠脉病变加重逐渐降低 ,但亚组间差异不显著 (仅P3∶0 <0 0 5 ) ;舒张功能参数亚组间差异显著 ,以时间常数为明显 (P <0 0 1) ,它同冠脉病变狭窄积分呈正相关 (r=0 72~ 0 78,P <0 0 1)。时间常数指数法值 (96 2 5± 17 0 6 )ms大于半对数法值 (6 7 94± 19 2 5 )ms,P <0 0 1;两者比同渐进压力呈负相关 (r=- 0 40 ,P <0 0 1)。结论 指数法时间常数是评价冠心病早期功能障碍较敏感。 Objective To study value of time constant evaluated on change of left Ventricular pressure during early relaxation Methods Sixty cases of coronary artery disease were diagnosed by coronary arteriography, hemodynamic parameters measured by left cardiac cather and left ventricular cineangiography Results LVEF decressed as lesion branches of coronary artery increased, but there were no significantly difference among subgroups (only P 3∶0<0 05); There were significantly difference of diastolic function parameters among subgroups, but T Value had a positive relation to stenosis scores ( r =0 72~0 78, all P <0 01) Texp (96 25±17 06)ms were longer than T ln (67 49±19 25)ms, P <0 01; T exp /T ln had a negative relation to asymptote ( r =-0 40, P <0 01) Conclusions Coronary artery disease imparies diastolic function prior to systolic function; Texp is a sensitive, accurate parameter evaluated early cardiac function impairment in coronary artery disease
