股骨干骨折内固定失效原因探讨

Analysis on the Failure of InternalFixation for Femoral Shaft Fracture

目的 :探讨股骨干骨折内固定失效的原因及预防。方法 :对12例股骨干骨折内固定失效病例回顾总结。结果 :失效原因为3例钢板窄小 ,螺钉长度不够 ;2例髓针偏细 ,髓腔扩大较宽 ,髓针尾端外露过长 ;10例为过早活动 ;4例为多发损伤身体太差 ;经治疗后12例完全愈合。结论 :合适的内固定材料选择 ,尽量少的手术损伤 。

Objectives: To analyze the failure causes of internal fixation for femoral shaft fracture and to search for prevention measures. Method: Summing up the data from 12 cases with femoral shaft fracture, who accepted the failed internal fixation. Results: The failure causes of the internal fixation for femoral shaft fracture in 12 cases were summed up. The failures were because of inadequate size of bone steel plate and inadequate length of bone screws in 3 cases, inadequate thickness and excessive length of broaches in 2 cases, premature functional activities in 10 cases and multiple damages in 4 cases. All the 12 cases were finally cured through remedial measures. Conclusions: The proper selection of materials for the internal fixation, the minimum damage during the operation, the proper external fixation after the operation and the guided functional activities are very important in reducing the failure probability of internal fixation for femoral shaft fracture.
