摘要 目的探讨胰岛素样生长因子1(IGF-1)对重症急性胰腺炎(SAP)大鼠肺损伤的影响及可能机制。方法Wistar大鼠72只随机分为对照组、SAP组、IGF-1组,每组24只。大鼠SAP模型采用5%的牛磺胆酸钠自胆胰管注入建立,三组在建模后6h、12h、24h各取8只测定血清淀粉酶、内毒素、二胺氧化酶、TNF-α、IL-1β、肺损伤病理评分,光镜观察肺组织病理变化。结果SAP组血清淀粉酶、内毒素、二胺氧化酶、TNF—α、IL-1β1、肺损伤病理评分较对照组升高;经IGF-1干预后,上述各项指标较SAP组均有下降。结论IGF-1可通过改善肠粘膜屏障、降低血液内毒素水平、减少内毒素诱导的炎性介质产生从而减轻SAP肺损伤。 Objective To evaluate the effect of insulin-like growth factor-1 (IGF-1) on the lung injury in rats with severe acute pancreatitis (SAP) and to discuss its possible mechanism. Methods 72 Wistar rats were randomly divided into three groups, control group (n=24), SAP group (n=24) and IGF-1 group (n=24). In each group 8 rats were randomly divided into 3 time units (6, 12, and 24-hour). The models of SAP were established by retro-grade injection of 50 g/L sodium taurocholate solution into biliopancrestic duct of rats. On the 6, 12 and 24-hour after establishment of models, serum amylase, endotoxin, diamine oxydase (DAO) TNF-α, IL-1β, histologic scoring of lung injury was determined. The tissues of the lung and small intestine were taken for light microscope observation. Results In the SAP group, serum amylase, endotoxin, diamine oxydase (DAO), TNF-α, IL-1β, histologic scoring of lung injury markedly increased. Compared with SAP group at the same phase, serum amylase, endotoxin, diamine oxydase (DAO), TNF-α, IL-1 β, histologic scoring of lung injury in IGF-1 group decreased significantly. Conclusion IGF-1 may alleviate the lung injury of rats with SAP by inhibiting the production of endotoxin, TNF-α, IL-1β and protecting the Intestinal barrier.
