应用脉搏波传导速度及瞬时波强技术联合评价初发高血压患者的动脉弹性 被引量:11

Evaluation of arterial elasticity in initial hypertension by pulse wave velocity and wave intensity technique

摘要 目的应用两种不同的方法探讨初发原发性高血压患者早期动脉弹性的变化。方法选择北京大学第三医院心内科门诊2011年3至10月的1—2级初发高血压患者作为高血压病组(n=32),年龄、性别匹配的健康志愿者为正常对照组(m=44),分别测量肱踝脉搏波传导速度(baPWV)及应用瞬时波强(WI)技术检测右侧颈总动脉。比较两组上述测量指标的差异。结果高血压组双侧baPWV值均明显高于对照组[右侧:(1472±198)m/s比(1353.4-233)m/s],[左侧:(1465±198)m/s比(1353±241)m/s],P〈0.05。高血压病组瞬时加速度波强(w。)[(9.8±3.8)×10。mmHg·m·s^[-3]比(7.4±2.4)×10’mmHg·m·s。]、瞬时减速度波强(w2)[(2.8±1.1)×10’mmHg·m·s^[-3]。比(2.0±1.0)×10。mmHg·m·s^[-3]]、僵硬度(B)[(6.9±2.6)比(5.1±1.3)]、压力应变弹性系数(Ep)[(99±39)kPa比(57±17)kPa]、单点脉搏波传导速度(PWV[3)[(6.0±1.1)m/s比(4.64-0.6)m/s]、WI脉搏波传导速度(冈WwI)[(6.24-1.4)m/s比(4.4±1.0)m/s]均显著高于对照组(P〈0.01),心电图R波顶点至W1顶点的时间(R-W,)[(100±14)ms比(103±13)ms]、w,顶点至w:顶点时间(W1-W2)[(261±20)ms比(274±15)ms]及顺应性(AC)[(0.84-0.4)mill。/kPa比(1.1±0.3)mm。/kPa]均显著低于对照组(P〈0.05)。结论肱踝脉搏波传导速度测量和瞬时波强两种技术均证实初发原发性高血压患者发病早期即已出现了动脉弹性下降。 Objective To explore the early changes of arterial elasticity in initial hypertension through two different methods. Methods A total of 76 hypertensive patients ( Grade 1 - 2, n = 32) were assigned into hypertension group while the age and gender-matched healthy population was selected as normal group (n = 44 ). All of them were monitored for baPWV in fight carotid artery by wave intensity technique. The parameters of two methods were compared between two groups. Results Higher bi-lateral baPWV were found in the hypertension group ( fight: ( 1472±198 ) rn/s vs ( 1353 ± 233 ) m/s), ( left: ( 1465±198) m/s vs ( 1353 ±241 ) m/s ) ( P 〈 0. 05 ). Compared with the normal group, Patients in the hypertension group had statistically higher W1 ( (9. 8 ± 3.8 )×103 mm Hg· m · s -3 vs (7.4 ±2. 4) ~ 103 ) mm Hg ~ m ~ s-3, W2 ( (2. 8±1.1 ) ~ 103 mm Hg ~ m ~ s-3 vs (2. 0 + 1.0) x 103 mm Hg ~ m ~ s-3 ), stiffness parameter(15) ( ( 6. 9 ~ 2. 6 ) vs ( 5.1 ~ 1.3 ) ), pressure-strain elastic modulus ( Ep ) ( ( 99±39 ) kPa vs ( 57±17 ) kPa), one-point wave velocity ( PWVβ) ( ( 6. 0±1. l ) m/s vs ( 4.6±0.6 ) m/s ), PWVWI((6.2±I.4) m/svs (4.4~1.0) m/s)(P〈0.01). R-WI((100~14) msvs (103 :t:13) ms), ( WI -W2 ) ( (261±20) ms vs ( 274±15 ) ms) and arterial compliance ( AC ) ( ( 0. 8 ±0.4 ) mm2/kPa vs (1.1±0. 3 ) mm2/kPa)in the hypertension group were statistically lower than those of the normal group (P 〈 0. 05). Conclusion The initial hypertensive patients may have impaired arterial elasticity during anearly stage.
