摘要
目的分析白内障行超声乳化人工晶状体植入术后低视力的主要原因,寻求防治措施。方法白内障超乳术联合折叠人工晶状体植入465例(478眼)术后低视力53眼,对低视力的原因进行了分析。以药物治疗、激光手术、屈光矫正及验配助视器等方法对低视力患者进行处理。结果术后随诊3个月~2年,视力〈0.05者3眼,0.05—0.08者6眼,0.1者5眼,0.12者10眼,0.15者13眼,0.2者7眼,0.25者9眼。其中3眼术后视力与术前相同,2眼低于术前视力,其余48眼术后视力均有提高。低视力为手术原因者占15.1%、非手术原因者占84.9%。经logistic多因素分析,最终进入模型的变量有4项,分别为年龄、术前矫正视力、术前眼部其他疾病及晶状体硬度。结论手术因素已不是白内障术后低视力的主要原因,而病例选择不当,如术前已存在眼部疾病等是造成低视力低的主要因素。
Objective To discuss the cause and management of postoperative low vision after phacoemulsification and intraocular lens implantation. Methods The clinical data of 53 eyes of postoperative low vision in 465 cases (478 eyes) with cataract phacoemulsification and intraocular lens implantation were studied retrospectively. They were treated with medicine, laser surgery, correction of refractive errors and ty- poscope for vision recovery. The follow-up was 3 months to 2 years. Results Fifty three ( 11. 1 % ) out of the 478 eyes developed low vision. The visual acuity (VA) was 〈0.05 in 3 eyes, O. 05 -0.08 in 6 eyes, O. 1 in 5 eyes, 0.12 in 10 eyes, O. 15 in 13 eyes, 0.2 in 7 eyes and O. 25 in 9 eyes. The postoperative VA was equal to the preoperative one in 3 eyes, below the preoperative one in 2 eyes, higher than the preoperative one in 48 eyes. The cause of low vision was surgical operation (15.1%), nonsurgical reason (84. 9% ). Multivariate stepwise logistic regression analysis showed that age, preoperative corrected visual acuity, preoperative ocular disease, nuclear hardness were the main cause of low postoperative vision. Conclusion The main cause of postoperative low vision after cataract phacoemulsification is non-operation factors.
