鼻内镜下泪囊支架植入术后局部反应分析

Analysis of inflammatory reactions after transnasal endoscopic lacrimal stent implantation

目的探讨鼻内镜下泪囊鼻腔吻合术（nasal endoscopic dacryocystorhinostomy，NES—DCR）联合泪囊支架置入术出现局部反应的原因、临床表现及治疗方法。方法对45例（45眼）采用NES．DCR联合泪囊支架置入术，1月后取出支架，术后随访半年至1年。结果45例，治愈39例，好转4例，有效率95．56％。出现炎症反应5例，占11．11％。结论泪囊支架运用于NES—DCR术可提高造孔的成功率，但也出现了局部反应，对手术操作，鼻腔的消毒，脓液的清理，术后的用药，泪囊支架的材料也提出了更高的要求。

Objective To discuss the cause, clinical feature and treatment of the postoperative inflammatory reaction ＇after transnasal endoscopic dacryocystorhinostomy （ NES-DCR ） combined with lacrimal stent implantation. Methods There were 45 cases （45 eyes ） with NES-DCR combined with lacrimal stent implantation. The stent was taken out one month after the operation. They were followed-up for half an year to 1 year. Results In 45 eyes, 39 eyes were cured, 4 eyes improved, the efficiency was 95.56%, Inflammatory response occurs in 5 cases, accounting for 11.11%. Conclusion Lacrimal stent used in NES-DCR technique can improve the pore success rate, but also appeared in inflammatory response. Surgical operation, the nasal cavity disinfection, clean-up of pus, postoperative medication, lacrimal stent material also put for- ward higher requirements.
