左卡尼汀对老年冠心病慢性心力衰竭心功能的影响

Effect of Levocarnitine on Cardiac Function of Elderly Patients with Coronary Heart Disease and Chronic Heart Failure

【目的】探讨左卡尼汀（LC）治疗老年冠心病慢性心力衰竭（CHF）患者心肌缺血及心功能的改善情况。【方法J76例平均（68．1±8．3）岁的老年冠心病CHF患者分为观察组（LC组）40例，对照组36例。两组常规治疗相同，观察组在常规治疗基础上给予LC3．0g／d，静脉滴注，疗程14d；比较两组治疗前后及组间左心室射血分数（LVEF）、心电图及胸闷症状缓解情况。【结果]LC组治疗后常规12导联中ST段压低总和及ST段压低〉0．5rflrfl的导联数均明显降低（P〈0．05），LVEF值显著升高（P〈0．01），与对照组比较差异具有统计学意义（P〈O．01）。观察组的显效率和总有效率均显著高于对照组（P〈O．05）。【结论】老年冠心病CHF患者联合应用左卡尼汀可以改善心肌缺血及心功能。

[Objective] To explore the improvement of cardiac function in elderly patients with coronary heart disease and chronic heart failure（CHF） after the treatment with levocarnitine（LC）. [Methods] Totally 76 elderly patients（average age of 68.1-8.3 years old） with coronary heart disease and CHF were divided into observation group（LC group, n -40） and control group （n - 36）. Both groups received the same routine treatment. LC group additionally received the intravenous injection with 3.0g/d LC for 14d. Left ventricular ejection fraction（LVEF）, electrocardiogram and the relief of chest tightness symptoms were compared between before and after treatment and two groups. [Results] In LC group, the sum of ST segment depression（EST） and ST segment depression~ 0. 5mm（NST） of 12-lead electrocardiogram were Obviously decreased （P G 0.05）, while LVEF was markedly increased（ P G0.01）, and there were significant difference between LC group and control group（ P G0.01）. The excellence rate and total effective rate in LC group were＇remarkably higher than those in the control group（ P G0.05）. [Conclusion] The combination treatment with levocarnitine used in elderly patients with coronary heart disease and CHF can improve myocardial ischemia and cardiac function.
