视盘内与邻近视盘视网膜下出血

Intrapapillary hemorrhage with adjacent peripapillary subretinal hemorrhage

报道2例视盘内与邻近视盘视网膜下出血病例,均系中国人,年龄分别为19岁及13岁,单眼累及,起病急。均有飞蚊症,视力分别为0.8及1.0。眼底除视盘内出血外,紧邻视盘鼻侧视网膜下亦有半月形暗红色出血。荧光血管造影出血区荧光遮蔽,后期视盘有轻度渗漏。门诊观察,2～3个月后出血吸收,未有复发,说明这是一良性自限性疾病。本文结合文献讨论视盘内与邻近视盘视网膜下出血的诊断、鉴别诊断及可能的病因。

Two cases of intrapapillary hemorrhage with adjacent peripapillary hemorrhage were reported. Both were Chinese, 19 and 13 years old respectively. Only one eye was involved with acute onset. Floater was the initial symp- tom. Visual acuity was 0.8 and 1.0 in 2 cases respectively. Fundus examination showed optic disc hemorrhage with adja- cent crescent dark subretinal hemorrhage. Hypofluorescenee at place where hemorrhage was located with late disc leakage was observed on fluorescence angiogram. Through observation, hemorrhage resolved in 2- 3 months with no recurrence. This indicates that the disorder may be a self-limited and benign disease. Combined with literatures＇ review, the diagno- sis, differential diagnosis and possible underlying causes of the diseases was disscussed.
