三维适形放疗治疗子宫颈癌术后盆腔复发或转移50例临床分析

CLINICAL EFFECTS OF THE 3-DIMENSION CONFORMAL INTENSITY MODULATION RADIATION THERAPY ON THE RECURRENCE OR METASTASIS OF CERVICAL CANCER AFTER OPERATIONS IN 50 CASES

目的探讨三维适形放疗治疗子宫颈癌术后盆腔复发或转移的临床价值。方法将100例子宫颈癌手术后出现盆腔复发或转移的患者随机分为研究组和对照组,两组各50例。研究组采用三维适形放疗加腔内后装治疗,对照组用15MV-X线,前、后2野全盆腔对穿照射加腔内后装治疗。结果两组3年生存率:研究组为96%(48/50),对照组为32%(16/50),研究组高于对照组(χ2=44.44,P<0.01);放射性直肠炎的发生率:研究组为6%(3/50),对照组20%(10/50),研究组低于对照组(χ2=4.33,P<0.01);放射性膀胱炎的发生率:研究组为8%(4/50),对照组为22%(11/50),研究组低于对照组(χ2=3.84,P=0.05)。结论三维适形放疗治疗子宫颈癌术后盆腔复发或转移,提高了生存率,降低了放射性直肠炎和放射性膀胱炎的发生率。

Objective To investigate the clinical effects of 3-dimension conformal intensity modulation radiation therapy（3D-CIMRT） on the pelvic recurrence or metastasis of cervical cancer after surgery.Methods 100 patients with recurrence or metastasis of cervical cancer after operation were randomly divided into the research group and the control group,with 50 in each group.The research group was treated by 3D-CIMRT with intracavity afterloading therapy.The other group was treated with the two-field whole pelvic irradiation with intracavity afterloading therapy by using the 15MV-X line.Results Two 3-year survival rates were 96%（48/50） in the research group and 32%（16/50） in the control group respectively（χ2 =44.44,P0.01）.The incidences of radiation proctitis were 6%（3/50） in the research group and 20%（10/50） in the control group（χ2 = 4.33,P0.01）.The incidences of radiation cystitis were 8%（4 / 50） in the research group and 22%（11/50） in the control group（χ2= 3.84,P=0.05）.Conclusion 3D-CIMRT can be used to treat the pelvic recurrence or metastasis of cervical cancer after operation to obtain the survival rate and to reduce the incidence of radiation proctitis and radiation cystitis.
