影响急性脑梗死患者病情进展的因素分析

The analysis of factors influencing the progression in acute cerebral infarction

目的寻找影响急性脑梗死患者病情进展的主要因素。方法前瞻性连续收集发病7d内住院急性脑梗死患者530例,选取可能影响病情进展的因素,对其在急性进展性脑梗死和急性非进展性脑梗死中的差异进行单因素分析,对差异有统计学意义的因素进行Logistic回归分析筛选出影响病情进展的主要因素。结果影响病情进展的主要因素为:大动脉粥样硬化、就诊延误、入院时血糖升高。结论不同病因急性脑梗死患者发生病情进展的危险性不同,正确处理脑梗死急性期高血糖,普及大众健康知识,对预防急性脑梗死病情进展、改善预后极其重要。

【Objective】 To study the risk factors significantly influencing the progression of patients with acute cerebral infarction.【Methods】 We consecutively recruited 530 patients with acute cerebral infarction,whose symptom onset was less than 7 days.Factors that may be related to progression were analyzed in 530 patients with acute cerebral infarction.Logistic regression analysis was used to determine the main factors to the progression.【Results】 The primary factors influencing the progression included large-artery atherosclerosis,hyperglycemia,hospital admission delay after acute ischemic stroke.【Conclusion】 The possibility of progression in acute cerebral infarction is different in different cause subgroups.In order to prevent the occurrence of progression in acute cerebral infarction and improve the prognosis of the patients,we should pay more attention to the patients with hyperglycemia,and to spread health knowledge in public.
