动静脉泵预防和治疗脑卒中合并肩手综合征 被引量:12

Prevention and Treatment of the Shoulder-hand Syndrome after Cerebral Stroke for Patients by Arterio-venous Hand Pump

摘要 目的观察动静脉泵对脑卒中后合并肩手综合征(SHS)的偏瘫患者的治疗效果。方法选取80岁以下脑卒中后合并SHS的偏瘫患者60例。在康复训练的基础上,将患者随机分为对照组和治疗组,治疗组给予动静脉泵治疗。5个月后评定上肢疼痛、水肿及综合性运动功能,比较2组患者治疗前后肘正中静脉的血流量和血流速度的变化。结果治疗后治疗组约85%的患者患肢局部水肿、疼痛及运动功能比治疗前明显改善(P<0.05);治疗组所有患者治疗后肘正中静脉血流量和血流速度较治疗前增加。结论对于脑卒中合并SHS的偏瘫患者,动静脉泵可以有效改善上肢水肿,有利于患肢的神经营养和功能活动恢复。 Objective To retrospectively investigate the effectivity of the arteriovenous hand pump treatment on improving arm congestion and edema of the stroke patients with hemiplegia,SHS,so as to evaluate whether it is beneficial for the upper limb nerve nutrition and function recovery activities.Methods A total of 60 cases of stroke patients under 80 years old with SHS hemiplegia were selected.Patients were randomly divided into 2 groups during rehabilitation training including treatment group and control group.Arterio-venous hand pumps were applied for five months in treatment group,but not the control group.The pain and swelling as well as changes of upper limb comprehensive sports function were evaluated,the biggest vein blood flow velocity(Vmax),each minute blood flow(BFV) changes in the middle of the elbow before and after treatment were measured and analyzed for both groups.Results The upper limb local swelling,pain and movement were significantly improved in 85% of the patients in treatment group(P 0.05).The vein maximum flow velocity elbow(Vmax),every minute blood flow(BFV) in the middle of elbow were increased in all patients of treatment group.Conclusion Arteriovenous hand pump treatment could effectively improve congestion and edema for stroke patients with hemiplegia,SHS,which could beneficial to the upper limb nerve nutrition and function recovery activities.
