摘要 背景动物实验和临床研究证实浅低温(32℃~34℃)对脑缺血神经元有显著的保护作用。但是,何时开始浅低温,以及浅低温需要维持多长时间(即浅低温治疗时间窗)才能起到最大限度的神经保护作用,尚无明确定论。目的综述脑缺血浅低温治疗时间窗的研究进展情况。内容阐述治疗时间窗概念及脑缺血浅低温治疗时间窗的研究进展;讨论早期浅低温和晚期浅低温脑保护的可能作用机制。趋向阐明浅低温治疗时间窗及机制,更好地应用浅低温于临床脑保护。 Background Animal experiments and clinical studies have shown that mild hypothermia (32℃-34℃ ) has significant neuroprotective effect against cerebral ischemia. The time (therapeutic time window) that when mild hypothermia begins and how long mild hypothermia persists for will determine whether neuroprotective effects are completely achieved. There is not an exact conclusion because the therapeutic time window is considered to depend upon the severity of the insult, depth of damage, and duration of cooling. Objective To review advances in research on therapeutic time window of mild hypothermia against cerebral ischemia. Content The concept of the therapeutic time window of mild hypothermia and the protection of mild hypothermia against cerebral ischemia is introduced. The mechanisms of early hypothermia and delayed hypothermia are discussed. Trend Clarifying these issues can be helpful to guide clinical treatment to cerebral ischemia.
