75岁以上高龄急性阑尾炎75例诊治分析

Treatment analysis of 75 cases of old patients with acute appendicitis above 75 years old

目的总结75岁以上高龄急性阑尾炎的发病情况,临床特点,治疗方案的选择,并发症的预防等。方法回顾近15年所诊治75例75岁以上高龄急性阑尾炎患者,手术66例,非手术治疗9例。结果治愈70例,好转4例,死亡1例。体会到高龄急性阑尾炎患者具有病程长,症状及体征不典型,坏疽穿孔及脓肿形成较多。结论须详细询问病史,全面检查,及时诊断,积极进行手术治疗,在可能的条件下术前充分准备,以减少并发症,提高治愈率。

Objective To summarize the incidence, clinical features, treatment options, and pre vention of complicationsof old patientswith acute appendicitis above 75 years old. Methods Review the diagnosis and treatment of 75 cases of acute appendicitis in patients over 75 years old, 66 cases in opera tion, 9 cases in non operation treatment. Results 70 cases were cured. 4 cases＇in improvement, 1 cases of death. Experience of senile acute appendicitis patients with long disease duration, symptoms and signs were atypical, gangrene perforation and abscess formation was more. Conclusions To require a detailed history, thorough examination, diagnosis, treatment of active operation, Under possible conditions of preoperative preparation, in order to reduce the complications, improve the cure rate.
